IMPROVING OUTCOMES THROUGH INNOVATIONS IN POPULATION HEALTH MANAGEMENT

FOCUS
I’m the lead consultant for community and population health for inHealth Strategies out of Baton Rouge, LA. We collaborate on innovative solutions to help communities and providers nationwide radically change the health outcomes for the populations they serve. Through leveraging community resources, key stakeholders, best practices, and cutting-edge ideas, we create healthier communities and achieve cost savings.

STRATEGIC INITIATIVE: Demonstration Project for Neighborhood Transportation Through Community & Stakeholder Collaboration
Implement a demonstration project of the power of community leadership, working with anchor institutions to transform a neighborhood. The long-term outcome will be the incorporation of multiple anchor institutions in similar work across the city. In year one, the goal is to establish a community-led leadership group to identify community needs and drive interventional action, particularly around food access, through the anchor and partner institutions, interweaving the transformational work with the broader, city-wide implementation strategy.

I’m a firm believer that not only can young people change our communities, but we must. We have to be involved, outspoken and organized. This program is providing me with more tools, training and networking to make a difference.
